Combatting Prison Recidivism

I'm looking for help in creating a call center system for former inmates needing services. Something that would serve as a hotline for resources for people that don't have easy access to computers or cell phones. What I'm envisioning is and 800# for people just released from jail and prison, needing services for anything, from housing and clothing, to job training and food. Specifically focused on people with no other support system in place, such as family or friends. I feel that there's a lot of organizations providing these services and I'd like to make it easier and quicker for people to get connected to these organizations. The ultimate goal is to help reduce the recidivism rate since part of the reason for recidivism is people are released without any resources.
